Rehabilitation services: for people 18 years and above who have complex and enduring mental health problems. The spectrum of Rehabilitation services covers;

Rehab & Recovery

People using these services will typically have had recurring and/or long admissions to acute services. Our aim is to help these people move on, potentially within 5 years.

Those using this service will generally require social and psychological support aimed at building personal coping strategies. The function of rehabilitation and recovery service is to take a longer term approach to building social networks and ordinary living skills. The overall aim is to empower people to actively engage with the services they require to maintain a valued place in the community.

Rehabilitation and recovery services are delivered in a range of small, domestic environments with 24 hour nursing presence and within our supported housing service which provides flexible support to people in a range of accommodation.

 

Supportive Rehabilitation

People using this service typically have had long associations with psychiatric services, commonly as in-patients. They will require ongoing support, potentially for the rest of their lives.

Those using this service will generally need more structured psychological support and/or physical assistance, and usually be over 50 years of age. The function of the supportive rehabilitation service is to take a long term approach to supporting people whose ability to maintain a valued place in the community is severely affected by their mental health problems and related experiences.

Supportive Rehabilitation services are delivered in as domestic environments as possible with 24 hour nursing presence.

 

 

The main features of the rehabilitation services are to support people in the management of their complex mental health problems. This is achieved through;

  • The provision of practical support e.g.

Helping with;

•  Money management

    • Domestic / housekeeping skills
    • Medication management
    • Personal care skills

•  The provision of psychological and social support e.g.

Helping people to;

•  Build and maintain relationships

•  Develop meaningful activities

•  Cope with the stresses and anxieties associated with developing independence / interdependence

•  Deal with the effects of their mental health problems

•  The provision of supportive physical and psychological environments

Based on;

•  Good quality housing

•  Personal space

•  Helping relationships

Supported Housing
Supported housing pic

Mental Health Concern established its Supported Housing as part of its rehabilitation and recovery service in 1997. The service provides support and ordinary housing in the Tyneside area with complex and enduring mental health problems.

The service promotes ordinary living and independence for tenants living in the community. It includes daily staff support in all areas of day to day living and the opportunity to access an on-call system operated by the Supported Housing team.
